UNIFI Helpline FAQ

You can submit a confidential, anonymous report using either:
– The Internet (through the secure Ethico website)
– The toll-free telephone hotline (available 24/7, 365 days a year)
Both options are designed to protect your confidentiality and anonymity.

The Unifi Helpline is designed for employees to report unsafe, unethical, or harmful situations; any violations of the Code of Ethics, or other concerns you may have.

When you observe some behavior that you believe violates our code of conduct, we expect you to report it. Ideally, you should bring any concerns forward to your direct manager, or other members of our management team. We recognize, however, that there may be circumstances when you are not comfortable reporting the issue in this manner. It is for such circumstances that we have established the Helpline. We would rather you report anonymously than keep the information to yourself.

We all have the right to work in a positive environment, and with that comes the responsibility of acting ethically and letting the appropriate people know if someone is not acting appropriately. By working together, we can maintain a healthy and productive work environment.

Yes. We certainly do. In fact, we need you to report. You may have initial knowledge of an activity that may be cause for concern. Your reporting can minimize the potential negative impact on the company and our people. Also, offering positive input may help identify issues that can improve corporate culture and performance.

Reports to the Helpline are answered by Ethico, an independent company that helps ensure reported concerns about unethical and illegal behavior. Reports are entered directly on the Ethico secure server to prevent any possible breach in security. Ethico makes these reports available only to specific HR individuals within the company who are charged with investigating the report, based on the type of violation and location of the incident. Each of these report recipients has had training in keeping these reports in the utmost confidence.

Yes. Ethico does not generate or maintain any internal connection logs with IP addresses, so no information linking your PC to the UNIFI Helpline is available. In fact, Ethico is contractually committed not to pursue a reporter’s identity. If you feel uncomfortable making a report on your work PC, you have the option of using a PC outside our work environment (such as one located at an Internet café, at a friend’s house, public library, etc.) through the Ethico secure website. Also, if you wish to be identified with the report, there is a section in the report for identifying yourself, only if you wish to do so.

No. Unifi strictly prohibits retaliation against anyone who raises a concern or makes a complaint within the company. It is expected that these concerns are raised within good faith efforts and not under false pretense.

When you file a report on the Helpline Web site or through a telephone call, you receive a unique ID and Password. You can return to the Helpline system again, either by Internet or telephone and access the original report to add more detail or answer questions posed by a company representative and add further information that will help resolve the issue. We strongly suggest that you return to the site within the time specified to answer any outstanding questions. If you filed the claim anonymously, you and the company remain in an anonymous dialogue seeking to resolve the situation.

The Helpline system and report distribution are designed so that implicated parties are not notified or granted access to reports in which they have been named.

The report is reviewed by Ethico and forwarded to the responsible company representative. A thorough investigation is conducted, which may include interviews and fact-finding. You can track the status of your report using the report number and password you receive.

Yes. Using your unique ID and password, you can return online or by phone to:
